Fatal Accident at Koidu Kimberlite Project

Koidu Kimberlite Project, Kono District, 26 September 2011: Koidu Holdings regretfully announces the tragic passing of Mr Yakuba Kondeh, a Trainee Articulated Dump Truck Operator who had joined the Company in June 2011.  The accident took place at the K2 kimberlite pipe open pit shortly after the afternoon shift assumed duty at 16h00 on Sunday 25 September 2011. It appears that the cause of death was drowning when the Articulated Dump Truck (ADT) he was operating plunged into a water-filled excavation next to the haul road in the bottom of the pit.

Operations were immediately halted and the incident reported to the Director of Mines, the United Mine Workers Union, Paramount Chief Paul Saquee, the Local Unit Commander of the Sierra Leone Police and the Senior District Officer of the Ministry of Local Government. The scene of the accident was immediately inspected by the Local Unit Commander, Senior District Officer and Ministry of Mines’ officials (including the senior Mines Monitoring Officer).

A full investigation has been launched by Koidu Holdings, supported by the formation of a Commission of Inquiry, consisting of the representatives from the Ministry of Mineral Resources, the United Mineworkers Union and Koidu Holdings.

Koidu Holdings’ Chief Operating Officer, Dino Coutinho, expressed sincere condolences to Mr Kondeh’s family, relatives and friends, saying “We are deeply saddened by this terrible loss of a valuable member of our mining team, and will take every measure to ensure that his bereaved family are supported through this time of sorrow”.

“This unfortunate event weighs heavily on the hearts and minds of all Koidu Holdings’ management and staff, who through the past eight years have maintained a zero fatality record related to our mining operations.  We give our assurance that we will continue to strive towards the highest health and safety standards possible.”
